You can only create a new Form activity if there is already a Form Template in the system, as the Form is created from the Template. If you do not have one, Create a Form Template first.

Go to Content Manager or Find Learning (see Access Content Manager or Access Find Learning if you're unsure how to do this).
Click on the Create tab near the top of the page.
Click on the Form button from the list of available Activity types.
Choose the Template you want to use by clicking the radio button next to its name.
Click on the Create button within the popup window.
The Form has been created and you will be taken to its administration area.
You can begin to assign Users in the same way that you would for Activities, Pathways, etc. by clicking on the Add users link in the Common Tasks area.
You can also click on the Users tab on the left-hand side, followed by the Register button, if you prefer.
If you would like to change the name of your Form, click on the Go to details link from the Common Tasks area (or click on the Details tab on the left-hand side).
Here you can change the Form name, description and objectives.
If the Form Template uses the Objectives field, you can specify which Form the past objectives are pulled from. This only applies if the Past Objectives setting in the Form Template is set to Manually add objectives only. When the User adds past objectives, the objectives shown will automatically be filtered to the Form specified here. For further guidance on using the Objectives field, see this guide.
Don't forget to click on the Save button near the bottom of the page if you make any changes in the Details tab.
Note: once a Form has been completed by a User, its progress cannot be reset like other Activities. A new Form needs to be created from the Template and allocated accordingly. For this reason, we recommend that you thoroughly test a Form before allocating it to Users.
