The template has been pre-populated with headings. Please do not delete these headings as the LMS reads these when the question bank is uploaded. Start writing your Survey questions in row 2. Each question must appear in it's own row without spaces (empty rows) in between.
We recommend using one Survey Question Bank for all your survey questions. You can easily search for and choose which questions you want to use when creating your Survey and it is not necessary to use all of the ones in your Survey Question Bank.
The following table explains what data is expected in each field:

This is a unique identifier to to uniquely identify this question within this template. Example IDs may be SQ001, SQ002 |
Type |
This is the type of answer required for the question. This can be either: FreeFormText (no spaces): an open question which allows for a comment style answer LikertScale (no spaces): a closed question which provides a list of single choice answers. For example, Strongly Disagree; Disagree; Neither Agree or Disagree; Agree; Strongly Agree. Range: a rating style question which invites the user to select a number along a scale. The minimum (min) and maximum (max) number are specified in the template along with the increment (step) the slider increases or decreases when moved. This question type will also provide a comment box allowing the User to explain their rating. Please enter just the text shown in bold, without spaces. |
Deleted |
This field is a mandatory and determines whether the question should be retired from the Survey Question Bank. To make the question available to use in Assessments, this should be No. However, if you no longer wish to use the question, you need to change it to Yes. This will remove it from the available questions when creating new Surveys. |

Category1-3 |
You can categorise your questions to make it easier to find them when creating a Survey. For example, you may have questions relating to the Trainer, the Venue or the Learning Objectives. As a minimum requirement you must complete Category 1 for each question. However, if you wish to further sub-categorise your questions you can use column 2 and 3 but this is optional. |
Once you've finished entering all your questions in your Survey Question Bank, Save the file ready to upload to the LMS.
