Once you have all your data prepared, you are ready to create your Fit and Proper Form Template. When the Form Template is ready you can then create a Form Activity. This is assigned to your Users for them, and any other workflow owner, to complete.

Fit and Proper Form Template
You might know exactly how you want your Fit and Proper assessment to look, but if you need a starting place why not start with our example template? You can use our Fit and Proper template to assess fitness and propriety in your organisation or create your own using Forms and Workflows.
Create Form Templates using Forms and Workflows:
If you would like to see or use our example Fit and Proper Form Templates, contact your Account Manager or Unicorn Help, who will arrange for the templates to be made available on your LMS.
Validation
Validation can be applied to fields in your Form Template. Validation defines whether a response to a field in a Form meets a predetermined requirement. For example, on a Fit and Proper Test, it may be mandatory that the User has completed the required CPD in order to be signed off as fit and proper.
When a Form which contains validation is being completed, fields which have met the validation requirements will be indicated by a green tick. If they have not met validation requirements (or the field has not yet been completed) a red exclamation point icon will show next to the field.

When the Form is progressing through the workflow, it can be rejected by a Manager or withdrawn by a User if invalid data is entered. If a Form Template contains any element of validation, Super Administrators can choose who should receive an email notifying them if the Form is withdrawn or rejected.
