Only Super Administrators have access to the functionality detailed in this guide.
Navigate to the User's profile. View the View Users guide if you are unsure how to do this.
Click on the Performance tab.
In the Current Forms section the User will see the latest forms that are in progress or have been completed. You will only see a form here once it has been initiated. To initiate a new form for the first time, a User should access the form from My Activities and a Manager should access the form from My Team Activity.
In the Previous Forms you will be able to view or print historic forms that were completed prior to any in the Current Forms section.
Note: the name displayed will be the Form Template name which may be different from the Form activity name.
Clicking on the arrow in the Status column will display who created the form and when as well as the last action carried out on the form.
There are a number of actions available to you under the Actions dropdown.

Will revert it back to a previous stage of the form. Click on it then choose which stage to revert back to from the dropdown list. |
Cancel | Will cancel the user's registration from the Form. All data entered into the form at any stage will be lost. |
Restart | Returns the status of the form to Not Attempted. Any data entered into the form will be lost. If a new version of the form has been published since they were originally registered, the next time the user initiates the form, they will be seeing the latest version. You can only restart a form if it is still progressing through the workflow. If the form is showing as complete, you will need to change its status before you can restart the form. |
Delete | Only available for previous forms. Will remove the form. |
You can view or print forms from the Performance tab.
Click on View to view a non-editable version of the form in the LMS.
Click on Print to view a .pdf version of the form which you can then print. The print version includes a header showing who the Form was assigned to, who it was reviewed by, when it was initiated and by whom, and, if completed, when and by whom.
